In line with other GP Practices in the area we are no longer offering Ear Irrigation. If you feel that your ears need unblocking, you will need to book an appointment with one of our GP's who, if appropriate, will refer you to the Hospital.

Forms to be completed by GP

Please note that if you require forms to be completed by your GP the turn around time is 3 weeks and depending on the type of form a Fee may be applicable. Please ask at reception at the time of handing in the forms for current prices.



Medication Reviews

It is extremely important that all Patients on repeat medication must have a medication review once a year.  Our staff are able to identify all patients who are due a review and will issue you with a form to complete around the anniversary date of your medication review. 

Once you have completed your medication review form and handed it back in to the surgery, it will be passed to your usual GP who will then check and review all of your current medications.  If everything is ok, the GP will re authorise your repeat medication for a further 12 months. 

Patients on certain medications may be required to have regular blood tests or blood pressure checks, the GP will inform you of this at the time of your medication review and you will be asked to make an appointment with our Practice Nurse or Health Care Assistant.

If you order your medication via the Electronic Prescription Service and you are due a medication review, please print off a medication review form and return it to the Practice by hand or post.

Please ensure that your form is completed and returned to the surgery as soon as possible to avoid unnecessary delays to further prescriptions.
